Vision Rejuvenation

There are many forms of vision rejuvenation options available to help you see more clearly, including everything from basic corrective lenses to advanced surgical refractive procedures. At Heritage Eye, Skin & Laser Center, we take advantage of advanced eye care technology to help ensure that our patients enjoy the clearest possible vision. Our doctors use laser vision correction and other alternatives to help our patients reduce their dependence on corrective lenses and enjoy life more fully.

Custom LASIK

LASIK is the most popular form of laser vision correction today. This safe, convenient, and effective procedure is appropriate for individuals who are nearsighted, farsighted, and those who have astigmatism. After LASIK, patients are no longer constantly dependent on glasses or contacts for clear vision.

Custom LASIK improves on the conventional LASIK procedure by customizing the surgery to the patient's unique eye structure. Each patient's cornea is different from all others; by taking into account the minute differences that are present even in patients with nearly identical prescriptions, Custom LASIK at Heritage Eye, Skin & Laser Center, is truly a customized and much more effective treatment.

Custom LASIK is a two-step process. In the first step, our doctor creates a small hinged flap on the cornea. In the second step, the flap is moved aside to allow for reshaping of the cornea to correct refractive errors. For years, LASIK could only correct standard prescriptions used for glasses or contact lenses. Today, with Wavefront technology, an advancement that Dr. Miselis uses, we are able to detect and potentially correct more than just standard prescriptions. We can identify tiny visual imperfections called higher-order aberrations that are individual to each and every eye. Until the advent of Wavefront technology, it was not possible to measure or correct these visual imperfections.

We also have the ability to utilize Iris Registration technology. The beautiful portion of your eye, the iris, is unique to each individual eye. The unique patterns on your iris are already used to identify people for security purposes. Iris Registration helps to align the laser treatment and may improve the outcome for some patients.

We use state-of-the-art diagnostic equipment to examine your eyes. We obtain highly accurate measurements that ensure very precise surgical outcomes. During the evaluation process we perform a test that scans the front of the eye and generates a 3D model of the cornea and the lens. We look for microscopic imperfections that can be corrected with LASIK but cannot be corrected with glasses or soft contact lenses.

Refractive Lens Exchange – An Alternative to LASIK

For those who are not a good candidate for LASIK, a lens replacement procedure (called Refractive Lens Exchange – or RLE for short) may be an option. RLE is a type of vision correction procedure in which the eye's natural lens is removed and replaced with a man-made intraocular lens (IOL). Alternative names occasionally used for RLE are clear lens exchange, clear lens extraction, or CLE. IOLs are designed to be multifocal, allowing patients to see clearly at a variety of focal depths and thus mimicking the natural function of the eye more closely than ever before.

Refractive Lens Exchange is essentially the same surgical procedure as cataract surgery, which is performed on approximately three million Americans each year. But with RLE, the eye's natural lens is not clouded—the clear lens is removed and replaced with an IOL simply to change the focusing power of the eye. Refractive lens exchange can reduce or eliminate large amounts of nearsightedness and farsightedness and therefore may be a better surgical solution than Custom LASIK for high prescriptions.

Because RLE is an internal eye procedure with the potential for more risks and complications than laser vision correction surgery, RLE is generally reserved for people who are not good candidates for LASIK, PRK, or other laser procedures or who have high amounts of nearsightedness or farsightedness that are beyond the accepted treatment range for laser surgery.

RLE can correct a wide range of refractive error due to farsightedness and nearsightedness, and RLE can also correct astigmatism if an astigmatism-correcting Toric IOL is used.

Advantages of RLE

RLE can effectively correct high amounts of nearsightedness and farsightedness that may not be adequately corrected by LASIK. It also is a viable alternative to LASIK for eyes with corneas that may be too thin for laser vision correction.

Custom Cataract Surgery

The use of Premium Intraocular Lenses (Premium IOLs) in cataract surgery has enabled us to customize a person’s optics to match their lifestyle. In other words, nearsightedness, farsightedness, and astigmatism can be corrected as part of the cataract surgery process to reduce glasses dependence. Premium Intraocular Lens (IOL) Options

Toric Lens Implant for Astigmatism

Most IOL implants are round or spherical in the way in which they bend the light to focus on the retina. Dr. Miselis, Heritage Medical Director and Surgeon, has had great success using a toric implant instead of a spherical lens for specific types and amounts of astigmatism. This implant incorporates an optical design that can correct up to several diopters of regular astigmatism. With the reduction of astigmatism during cataract surgery, patients can see better at distance or near and are much less dependent on prescription glasses.

At the time of your RLE or cataract surgery assessment, please ask if you would be a candidate for this type of implant.

Multifocal Lens Implant

Standard lens implants have one focal point, meaning they focus for vision at one distance only (near or far). Multifocal implants offer the possibility of seeing well at more than one distance, with less dependence on glasses than the standard monofocal lens implant. As their name suggests, multifocal IOLs have more than one lens power. They work on the same principal as progressive eyeglass lenses and multifocal contact lenses — different zones in the lenses have different lens powers to improve vision at all distances.

At night, unwanted glare and rings around lights can bother some patients. This is rarely a problem with the current generation of multifocal implants. Many patients with no or minimal astigmatism are glasses-independent after the procedure. It is not possible to promise every patient that glasses won’t be necessary after surgery. However, in the studies submitted to the FDA, more than 80% of patients stated they no longer needed to wear glasses after having a multifocal lens implant placed after surgery. Those that did need to wear glasses were patients who needed glasses for intermediate vision only, such as for reading a computer or reading music.

At the time of your RLE or cataract surgery assessment, please ask if you would be a candidate for this type of implant.

Accommodating Lens Implant

The accommodating lens has hinges on both sides of the IOL, which can be moved by the ciliary muscles, allowing the possibility that the eye will focus more naturally at a greater range of distances than traditional lens implants. The accommodating lens corrects vision for near, intermediate, and distance. The advantage of this lens implant compared with the multifocal lens implant is less night glare. Most patients, however, still will need reading glasses for small print or for viewing some objects close up.
